Every athlete in CISD is required to have an Athletic Medical Information Form (aka – Physical Packet) on file with the Athletic Training Staff of the Athletic Department.  This is a yearly requirement.  There are 2 options to completing the packet:

  1. Pick up the 12 page paper packet from the coach, Athletic Office, or Athletic Trainer.
  2.  Complete the online physical form by creating a parent account and entering all of the medical / UIL information into our RankOne system.  You will need your child’s student ID number to proceed.  Once this portion is completed, you may print the 4 pages required for signatures and notary.  DO NOT PRINT THESE PAGES UNLESS YOU HAVE COMPLETED THE ONLINE PORTION FIRST.

After you have completed one of the options above, you will be required to take the child to the doctor of your choice for the physical.  The physical must be signed, stamped and dated for us to accept it.  After the doctor’s portion is complete, you will bring in the paperwork along with your 2 proofs of residence and a valid ID.  The only accepted proofs are as follows:

Light bill, gas bill, water bill, mortgage statement, rent or lease agreement, or property tax statement (not income tax).  We will not accept garbage, cable, phone insurance or medicaid.

We try to make this process as easy as possible for our parents.  Every year in April, we provide at least 4 Notary Nights.  This is when you will come and complete the NEW athletic form with the current UIL forms, either online or a paper packet.  The packet will be checked for accurateness and you will have it notarized before you leave.  the 4 nights are from 4:30 pm – 8:00 pm in order to give the parents a variety of days and times to get this done.

We also provide a doctor to come to our facility and administer an athletic physical examination.  You can pay the cost of $20.00 and your child will be included in the list of exams.  We arrange for our doctor to come to our facility in May.  One day is specifically for junior high and one day is for high school.  Any junior high athlete that has paid the fee will be brought to the Bill Neal Center via bus.  The parent will pick up the athlete upon completion of the exam.  Any high school athlete will come over with the coach during athletic period or immediately after school.  This process keeps our athletes on our “May to May Rotation” so they will not miss any aspect of a sport because the physical is expired.  It is especially beneficial to our parents because there is no need to take off work to sit in a doctor’s office.

We strongly encourage the parent’s to utilize our system.  Waiting until the day of tryouts in detrimental to the athlete because the student will not be “clear” and may delay the eligibility to try out.
